Music in Healthcare Settings Conference: Derby, 16 July 2015

We are delighted to launch the forthcoming International Music in Healthcare Conference, hosted by OPUS Music CIC in partnership with Royal Derby Hospital and Air Arts to Aid Wellbeing.

Bringing together music for health practitioners, healthcare staff, promoters, funders, researchers and other key stakeholders, this event promises to provide stimulus for discussion and debate around the ongoing development of Music in Healthcare settings across the UK and beyond.

A mix of thought-provoking presentations and discussion groups throughout the day will leave all stakeholders with new contacts and new ideas for continuing to develop their own practice.

We are also hosting a Music in Healthcare Settings ‘Music Sharing’ day on Friday 17th July 2015, to be held in Derby. Any musicians attending the conference are invited to come along from 9.30am-3.30pm (stay for as long or short a time as you like!) to make music with like-minded musicians (small charge of ÂŁ2 payable on the day to cover refreshment costs).

Continued music-making residencies in children’s hospitals

We are delighted to be able to continue, and expand our existing relationships with Derbyshire, Leicester, Nottingham and Kings Mill Children’s Hospitals with support from Youth Music and from our local and hospital partners.

This support means that OPUS musicians will now visit each hospital on a weekly basis until April 2016, making music for and with the patients, their families and staff.

Our recent evaluation and documentation work has demonstrated the highly valued impacts of this practice; we are incredibly grateful for the support of all our partners in enabling this work to continue.

Evaluation: OPUS in Children’s Hospitals

OPUS’ current music in hospitals programme, taking place in Derbyshire, Leicester, Nottingham and Kings Mill Children’s Hospitals, is coming to an end in March 2014. We hope to resume the practice in May 2014, funding permitting! As part of the current programme, OPUS engaged the services of Evaluation Consultant Dr Anneli Haake to evaluate the programme. Anneli has done a fantastic job, capturing evidence of the impacts of this practice and pulling this all together into academic and summary report documents, an A1 size academic poster (for display in hospitals) and the film (3 versions) previously shared here. Sunshine in My Heart Film: OPUS in Children’s Hospitals

We are delighted to share our film, made as part of the evaluation process of our current Music in Children’s Hospitals practice supported by Youth Music, Derbyshire Hospitals Charity, Nottingham Hospitals Charity, Leicester City Council and Nottinghamshire County Council. The film was captured at Nottingham and Leicester Children’s Hospitals by our fantastic external evaluator Dr Anneli Haake.

From 11th-17th April, we have been busy delivering our latest 5-day training course for musicians, exploring the skills and competencies required of a musician to work in healthcare settings.

Over the course of five days, 8 trainees and 3 trainers have worked together to develop their skills, knowledge and awareness and visited the Derbyshire Children’s Hospital to make music with patients, visitors and staff on medical and surgical wards, in Neonatal Intensive Care and in Children’s Accident and Emergency.

All found the training experience to be stimulating, exciting and hugely developmental. We would like to thank our fantastic group of trainees for their music, their passion for this practice and for their deep reflection which has helped us all to learn and to develop.

Thanks also to our funders and supporters of this practice, including the Derbyshire Children’s Hospital, Youth Music and Derbyshire Music Partnership for making this training course possible.

“Can we sing another song?”

Working in Derby Children’s Hospital with Nick Cutts and Rich Kensington today we played our way along the upper wards in the morning. We met a young patient who was so relaxed and open about her music and singing, she just couldn’t get enough of making music with us.

We started with “Yellow Bird” as an instrumental piece and then a sung song. The patient and another friend in hosptial were both offered a shaker instrument to join in with – which they both did with great joy in their faces – sometimes fast, and then slow, and then stopping to listen to the music on its own.

I smiled at the patient and asked if there was anything that she liked to sing – to which the answer was certainly “yes”. She said her favourite was “The Wheels on the Bus”. Oh great we know that one – and we sorted out which verses we could cover. This young patient, lying on her back for medical reasons, unable to sit up, joined in and led the song, thinking of new different verses with Mum’s help. Her voice was quite confident and happy and clear, such that a group of doctors standing nearby on their rounds, stopped and looked up from their work.

“Can we sing another song?” came her voice, at the end of that one…..we decided on Five Little Speckled Frogs, and encouraged her to help with the counting. She was not quite so confident with the words on this one, but none-the-less made every effort to join in with us.

She then became interested in my violin and I moved around the other side of her bed and showed her the instrument. She plucked the strings with such care and sensitivity. Then I explained how to touch the bow, and with Mum’s hand on one end of the bow, hers in the middle and mine at one end, we drew the bow back a forth across the D string.

“Can we sing another song?” she asked, whilst still bowing the D string on the violin…..Twinkle Twinkle in D major of course, worked really nicely. She played so gently and sang the words at the same time.

So lovely to meet this young patient – to listen to her voice and share those musical moments. The fact that we were in hospital did not really matter – the fact that she was lying down didn’t seem to hinder her participation and indeed, leadership in the music.

We did indeed sing another song.

Taking time to be with people

A big part of our work as musicians in hospitals is to be there as a musician and a human being spending time with other human beings, to make music for and with them and to create a cultural venue within a clinical environment.

We often get asked if it is emotionally difficult to spend so much time in hospitals, especially with children. There are, of course, always emotional moments in our practice, and we allow ourselves to be emotional as part of our professional practice, indeed it is important that this emotion becomes part of our music-making to allow us to be ‘in-tune’ with the patients, visitors and staff with whom we work. We must always be careful, however, that we do not project our own emotions onto others – this is also part of our professional undertaking.

It is, however, more normal for us to work with the well-part of the person, to enhance and support the cultural, vibrant, and well part of the person. Medical staff work hard to take a holistic approach to their work, however, they are principally there to treat the illness. We are hugely privileged to be able to take the time for this approach, one which seems to complement and support the work of the hospital staff so well.

Over recent weeks, we have seen this human-to-human approach work so well. In Sheffield Teaching Hospitals, working with elderly patients and those with spinal and neurological injuries, we have recently been able to take more time to explore musical interests with individual patients, to support them in singing and playing musical instruments with us, and to rediscover their creative and cultural selves. Medical staff have observed and taken part in some of these sessions, making new, human-to-human contact with patients and seeing new potential despite their illness or injury. It is fantastic to be able to support these new patient-staff partnerships.

In children’s hospitals, the time spent with individual patients and their families becomes special time, time for a parent and new baby to bond in a neonatal intensive care unit, time for ‘normal life’ to resume if only for a moment, time for music 🙂 When doctors, nurses and other hospital staff become part of these interactions, the space and relationships within the hospital change completely, and we are all human beings together.

Leo Tolstoy wrote (What is Art, 1897):
‘… In order correctly to define art, it is necessary, first of all, to cease to consider it as a means to pleasure and to consider it as one of the conditions of human life… Art is a human activity consisting in this, that one man consciously, by means of certain external signs, hands on to others feelings he has lived through, and that other people are infected by these feelings and also experience them.’

It’s wonderful to be able to share music-making, a ‘condition of human life’, with all those we encounter in hospitals.

It’s clear from the great feedback we are receiving that our aim of creating cultural venues within the hospital is really working, allowing children, parents and staff to share music-making together, and within communities of wards, bays and families. Thanks everybody for your feedback, including some fabulous pictures which I will try to post soon 🙂

A Good Musical Experience – Wednesday 21st Nov 2012

Nick and I began playing in the Sunflower waiting area – three children aged around 8 years of age were waiting, a little bored, and seemed eager to join in.

We sat down and played a tune for them to listen to – they gathered round immediately – Nick began to give out percussion instruments.

We turned to Gallopede (one of our favourites for facilitating participation). Each child joined in with their own rhythmical pattern with the music – Nick led them for a while and showed a slightly more complex patttern to try.

We changed the dynamics – we led – they led.

The tempo changed – with different leaders here again.

Nick tried a different style – could we slow things down and change the mood with a waltz? We played Valse vor Polle – and the children changed what they were doing and matched the style.

Finally our musical friends found our box of musical instruments and had a little session exploring some of the sounds that could be made on each of them – including trying out my violin and bow.

I felt that the interaction had been successful and complete, and thought it best to start packing the instruments away, with their help. It is always good to have a thought for when a session should finish – before anyone gets overexcited – end with positive experiences.

This felt like a really good all-round musical experience and it appeared as though the children involved had really been engaged and enjoyed it for about 20 minutes.

Chickens and Frogs

On 7th Oct, as part of our usual Wednesday work around Puffin Ward in Derby Children’s Hospital, we overheard a young boy in isolation, crying and thoroughly focussed on the marks that appeared to cover his body, itching and quite distressing him. He had toys all over his bed that seemd to be new – still to be unwrapped – but not distracting him from his discomfort.

We started playing an upbeat version of the “Valse vor Polle”, in 4 time – this did not seem to connect with him. Richard suggested we play something different – something with a gap in it…….an interesting suggestion – why would the space in the music grab someone’s attention more than continuous sound?

We started to play “Who stole my chickens and my hens?” leaving a rest in the music after each line of question in the song. After about the second time, he turned to look at us playing through the doorway. Still crying, but with gaps in his noise now, so he could hear when the music continued. Slowly but surely, his crying sounds resided and he started talking to his Mum about tthe chickens in the song – what a silly song!

There came a request – something about a frog? Nick fetched our speckled frog guiro instrument and we started playing and singing “Five Little Speckled Frogs”. We had only got as far as frog number 3, and he was smiling and opening his new toys, calmly and more relaxed.

We left them smiling, playing with new toys, and with their atmosphere changed at least for the time being. It appeared as though this young man was sufffereing with chicken pox, and we had sung songs about speckled things and chickens – only we saw the funny side to this – he did not notice at all, but seemed to benefit from the change in his day.

 

Music in Healthcare Settings Training

It’s the final day of our 5-day music in healthcare training today, and we’ve had a great time exploring the skills and competencies required to deliver this work alongside a fabulous group of trainees. We’ve spent time in the training room exploring, alongside many other things, repertoire, improvisation and ethical principles behind this practice. We’ve also spent a couple of afternoons at Derbyshire Children’s Hospital, playing music for and with patients, visitors and staff on surgical and medical wards, in the neonatal unit and in A&E. Final day today bringing all our learning and experiences together. Many thanks to our trainees for embarking on this journey with enquiring minds, a spirit of adventure and enthusiasm, and a real passion for this practice.

Shadowing OPUS musicians in hospital

I spent two consecutive Fridays shadowing the Musicians from OPUS on the children’s wards at the Royal Derby Hospital. On the first Friday the instruments that the musicians performed with were the guitar, a bodhran drum, ukelele and the melodeon. The second Friday the melodeon was replaced by violin. The musicians would travel from ward to ward performing a mixture of songs, such as “sunshine in my heart” and “wind the bobbin up”, as well as an array of instrumental pieces.

We also brought a trolley filled with various percussion and melodic instruments, these instruments were dispatched to the children whenever a musical connection was established. The musicians also had a designated play area on one of the wards where these instruments were laid out and the children were able to explore and express themselves whilst playing with the instruments.

Whilst observing the musicians in these various settings I was able to witness how the personalities of the children impacted on the music and vice versa. For example: when they were working in the designated play area, one particular child cautiously started playing with the instruments, warily experimenting with the volume and sound of the instrument. As the child interacted with the instruments they seem to be testing the musician’s reactions. This opened a gateway of communication for the musicians, allowing them to instil confidence in the child’s expression with the instrument. The musicians achieved this by mimicking back to the child what they played on the instrument. It was interesting to observe how some children became excited and others became more thoughtful in their musical expression after this communication had taken place.

Another interesting observation I made whilst shadowing the musicians as they travelled around the wards was, how the songs and the instrumental pieces produced different effects. The instrumental pieces seemed to create a deeper connection with the children and their families. On many an occasion emotions were unveiled by the instrumental pieces. Whilst the songs seemed to act as more of an interlude, almost having an element of play attached to them.

As the musicians went from areas of play to, areas where children were suffering with serve illnesses, I was able to witness how the musicians musically sensed the environment. They seemed to know whether a song or an instrumental piece was required in order to connect with the children or individual child. This displayed a deep sensitivity to the children’s physical and emotional situation. It was a very powerful and positive experience that has left me with many new questions and profound memories.

New music in health residencies

As you’ve probably noticed from our lack of blogs recently, OPUS is currently on a summer break. While we’ve not been in the hospitals so much over the past month, much has been going on behind the scenes to finalise arrangements for our new residencies beginning very soon. Thanks to funding and support from Youth Music, Nottinghamshire County Council and Leicester City Council, we are delighted to announce new long-term residencies in children’s wards at three new hospitals beginning in September. These will take place at Leicester Royal Infirmary, Queens Medical Centre, Nottingham and Kings Mill Hospital in Mansfield, with OPUS musicians visiting the hospitals on a weekly or fortnightly basis. This is planned to continue until at least March 2014, though we hope to sustain these residencies beyond this date. We are extremely grateful for the support of our partner hospitals and our funding partners in making these new residencies possible. OPUS has busy and exciting times ahead, with existing residencies continuing at Derbyshire Children’s Hospital and Sheffield Teaching Hospitals alongside training programmes for musicians and healthcare staff. We’re all really looking forward to it!

Friday 20th July

We began our morning’s work in the Sunflower Ward, as has been our habit for the last few weeks. We trialled a new tune “Astley’s Ride” which worked well in 4 time with the potential for patients to join in and turned into a waltz quite easily too – much to explore with that one in the future?

We entered the post / pre op ward behind the reception area with some caution. A young boy was being asked to take some medicine by his father, and he appeared worried about this prospect. A mother lay curled up in a chair, waiting for the return of her child. A three-year-old girl sat with her mother, obviously distressed and possibly feeling sick. Another little girl sat with her parents at the end of the room, and a teenaged girl lay apparently sleeping. This was quite an array of different situations presenting themselves at the same time in the one room, so some careful consideration was needed as to what music to play and with what character and volume.

We chose “Mari’s Wedding”, normally a jolly polka, but we started it as a relaxed and calm waltz, knowing that if the mood required it we could pick up the pace and change the dynamics according to the space. We played around the tune and improvised the melody and structure about 5 times as we slowly moved down the room. However, it did not seem appropriate to change the tempo and mood, so we remained in three time.

The little girl at the end of the room with her parents engaged with us and locked an intrigued stare with me as i played the piece and moved towards her, the teenager awoke slightly and lay in bed listening. The little boy relaxed and took his medicine and found something else to play with. The distressed little girl cried more, cuddled with her mother, relaxed and almost drifted off to sleep by the time we left the room.

It was only a matter of minutes that we were in that space, but it felt as though our music had provided an opportunity for the mood of the whole room to transition from distressed to relaxed; from unrest to calm. Hard to quantify something like this, but I was moved to tears on exiting the room.
